The Standard PPO option offers a combination of deductibles, copays and coinsurance through in-network or out-of-network providers. For most eligible care, you must meet the annual deductible before the plan begins paying its share of the cost. You can also contribute to a Health Care FSA to pay for eligible expenses. In this plan you’ll pay higher paycheck contributions, but you’ll have a lower deductible and out-of-pocket maximum.
